- Your full name as it appears on the insurance card.
- Your Member ID Number and Group Number (if there is one)
- Your date of birth.
- The 800 number for member or provider services.
Payment
Cash, check, all major credit cards and Venmo accepted for your share of the payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
